“A Patient-Centered Medical Home” is where you come to receive healthcare services that are personalized to you. If you need a service we do not provide, we work to ensure you still receive this care by providing referrals to outside providers and specialists.
We are also able to connect our patients with community resources, even those not relating to health. To us, part of being “healthy” is having the resources you may need to care for yourself. These include safe and stable housing, nutritious food, financial stability, and a life without burdens of abuse or extreme stress. If you or a loved one are in need of community resources, please let us know at your appointment, and we will be happy to assist you.
Part of our mission and values is to provide “accessible, affordable comprehensive primary and preventative health care services to the residents of Richland Parish [and surrounding areas] without regard to ability to pay. We accept most private insurance as well as Medicaid, CHIP, and Medicare. As an accredited Federally Qualified Health Center, we aim to work with our patients to create healthy families and a healthy community. Our Patient Assistance Program is here for you if you are concerned about your insurance status or ability to pay for your medical care. Please ask about our Sliding Fee Scale.

Main Clinic
Our main clinic is found across the street from Delhi Hospital & Delhi Rural Health Clinic. Please enter through the front doors.
Dental Clinic
The Dental Clinic is located in a white brick building next to the main clinic.
WellPath Solutions
WellPath Solutions is located at the same address as our main clinic. The entrance is separate and can be found on the right side of the building.
