The Delhi Community Health Center, Chronic Care Management Program was created to make sure that our patients get the best care and support possible. After becoming a VIP (Very Important Patient), you will have access to a multitude of benefits including:
Concierge Services
Managing medications can be challenging. Managing appointments or rescheduling can be time consuming. Remembering all that you wanted to tell your provider can be tricky. We can help with this and will do our best to take the stress out of managing your health care!
Advocacy
We want to provide care for you even when you are not in the clinic. We will provide support, education and a customized care plan to assist you on your journey towards optimal health.

Becoming a Delhi Community Health Center Patient allows you to receive all we have to offer. We strive to be “Your Patient-Centered Medical Home”—a trusted partner in your journey to better health.

We are also able to connect our patients with community resources, even those not relating to health. To us, part of being “healthy” is having the resources you may need to care for yourself. These include safe and stable housing, nutritious food, financial stability, and a life without burdens of abuse or extreme stress.
We accept most private insurance as well as Medicaid, CHIP, and Medicare. As an accredited Federally Qualified Health Center, we aim to work with our patients to create healthy families and a healthy community. Our Patient Assistance Program is here for you if you are concerned about your insurance status or ability to pay for your medical care. Please ask about our Sliding Fee Scale.
